Open Gate Transitional Services of Florida, Inc. is here to stand in the gap. Our Case Submission Intake Form allows families and advocates to document these injustices, so we can help you be heard, supported, and connected to the right channels for change.

 

This form is currently for residents housed in Florida, but we are working hard to expand our support nationwide through the Voices Unheard National Submission Network.
