How to Submit a Case to Open Gate

At Open Gate Transitional Services of Florida, we understand how overwhelming it can feel when your loved one is wrongfully punished or mistreated inside the prison system. That’s why we created the Voices Unheard: Case Submission Intake Form — a confidential, organized way to help you speak up and help us advocate effectively.

Complete the Case Submission Intake Form
Visit our website and click “Submit a Case” in the top navigation bar. Fill out the form with as much detail as possible:

Your full name & contact info

Your loved one’s full name, DC#, and facility

A summary of the issue (false DRs, confinement without due process, gain time loss, retaliation, etc.)

Dates and documentation (if available)
✅ TIP: You don’t have to be the incarcerated person – family members and approved advocates can submit on their behalf!

We Log, Track, and Organize the Case
Once submitted:

Your case is logged into our secure tracking system

You’ll receive updates, reminders, and any needed forms (via email)

We assign your case to the appropriate Regional Expert if applicable
Your submission will be stored in a personalized case folder that our team uses to track timelines, paperwork, and updates.

You may receive:
